2006 CONTEST RULES

Activities
duplicated from copyrighted material or previous award winning
entries from the National Awards Competition for Adult Day Centers
are not eligible for this competition.
2. For
each
entry include at least one supporting document or material that will
help describe or show your entry to the judges, e.g. a photograph or
sketch for each application that involves art/craft activities or
equipment for games, newspaper clippings, videotapes, etc. DO NOT
SEND ORIGINALS. All submissions become the property of Creative
Consulting for Adult Day Centers and will not be returned.
Furthermore, by submitting the contest entry you grant Creative
Consulting for Adult Day Centers the right to use your entry or an
edited version of same, in any of their publications including but
not limited to,
The Information Source for Adult Day Centers®.
3 Centers
are responsible for blinding all of their entries. The
only
place the center’s name or location should appear is on the cover
sheet for the application package. Supporting documents should be
labeled with the name of the
entry
rather than the name of the center.
4 Complete
a separate application form for each entry. Send
FIVE
copies of each completed award application under one cover sheet.
Centers are limited to five different entries that can be submitted
for awards. Multiple copies of support documents are not required
since they will be rotated among the judges.
5 All
mailed applications must be
postmarked
by March 15, 2006. Emailed entries must be received by 5:00 EST on
March 15, 2006. Faxed applications will not be accepted.
6 Prizes.
One-year newsletter subscription ($48 value) plus $100 to first
place winner of each category; one-year newsletter subscription ($48
value) to second place winner of each category. Winners will be
announced beginning in the June 2006 issue of
The Information Source for Adult Day Centers®.
7 Refer
to the descriptions of each award category in this brochure to
select the appropriate category for the entries submitted. Each
category will be judged on the criteria as specified on the
application. Judges reserve the right to judge entries according to
the most appropriate category. No purchase is necessary.
8 Submit
the application packet containing the FIVE copies of each
application and any support documents with
one
cover sheet [letterhead works great]. The cover sheet should

CATEGORIES
Therapeutic Activity Award
(General Audience):
It is not the end product that necessarily makes an activity effective.
The difference made in how participants respond in the moment as well as
over the course of time should influence whether or not an activity
should be repeated. Entries in this category should be appropriate for
a general audience and might include exercise, children, a holiday,
horticulture or music.
Fundraising
Award:
Successful
businesses secure necessary funds to operate cost effectively, expand
services and provide quality programming. Entries in this category
should be designed to increase the cost effectiveness of the business,
and probably also enhance the visibility of the program in the
community, state or nation. Examples may generate operating revenue
(based on fees for service) or non-operating revenue (donations, grants,
etc.)
Therapeutic Activity for Targeted Populations
(Dementia or Other):
Every person who
attends an adult day center brings a special interest and need. Entries
in these categories should include an activity or program designed to
improve the well-being of participants with specific illnesses or
impairments, e.g. stroke survivors, persons with dementia, multiple
sclerosis (MS), visually impairments, mental retardation, developmental
disabilities.
NEW!
Adult Day Services Hero:
These workers serve as a model of commitment to quality and
compassionate care in the industry. Any paid professional caregiver or
team of caregivers, who has direct contact with adult day program
participants and/or caregivers, is eligible.
Judging criteria for each category are specified on the
application.
